Not your ordinary Russia, he is Demon!Russia, meaning, he has mechanical claws protruding from his back, but they're retractable. Thing is, this version of Ivan is much smaller and younger; though the same bit dangerous, perhaps more. He has the same demented thoughts; actions, etcetera.
Instead of a waterpipe, though, he carries a shovel, and instead of having normal violet eyes, his eyes are pitch black and big, and in the center of his eye is a slitted white pupil. His appearance making him much more, well, bloodcurdling, he's even scarier with his voice. He has a mature voice for such a young looking youth; he has the same voice as Russia, though he has a heavy Russian accent unlike normal Russia.
As of now, as Demon!Russia, he's 13 physically. (Yes, he drinks vodka at 13. Lovely, right?)
Referring to the Russian Plague Epidemic of 1771, before he murders his victims with his shovel, claws, or perhaps bare hands, he recites the nursery rhyme (in his own words) "Ring Around the Rosy":
"Kol kol kol,
Ring around the rosy,
Pouches full of posy,
Blood, blood,
They all stop breathing."
When Russia sings about the blood part, he refers to septicemic plague, since one of the symptoms are bleeding from the mouth. As when the boy refers to the breathing verse, he means the third and most dangerous type of plague: pneumonic plague, since that type of plague affects your lungs and breathing, causing death after just one day.
The reason why Ivan is so demented and evil, is because of a horrible history of bloodshed, war, communism, and pain. His country is filled with Russian mafia, and yes, it may be a very pretty country, but because of war and loads of rules, he mentally cracked. He yearns the lifestyle of being in the country, surrounded by sunflowers and staying in isolation and dying with a sunflower.
Russia wears a scarf around his neck, gifted to him from his sister Ukraine.
The boy seems to be on good terms with Iceland, Canada, and well, perhaps China, but he just stalks China all the time. No one knows exactly why; some think he wants to murder China, though he doesn't at all.
Though he's come to the conclusion that he's beginning to spook the smaller country by his following.

Much like Rena (from Higurashi No Naku Koro Ni), he's a yandere. He shows a sweet, cute exterior, always smiling and being polite, though inside he constantly thinks about how fun it'd be to just smash everyone's head with his shovel.
He can't help but think about being lonely, then. If he killed everyone, he'd be lonely, and being lonely isn't fun. He wouldn't have Latvia to tease, he wouldn't have Lithuania to boss around, he wouldn't have his nice sister Ukraine, and he wouldn't even have China to stalk.
Despite his scary energy and vibrations, he's simply hollow and alone, and he can't deal with it, so he's become insane. If you listen to his theme song, Winter, it will explain a lot about him. Read the English lyrics while listening to the music at the same time; about how it starts off with a music box and goes into death metal like his personality.

Referring to his history of bloodshed and communism, this is explaining how others should become one with Russia so they would not get hurt by him and instead protected; but unfortunately for Russia, everyone seems scared of him and they refuse his offers.
In the verses that talk about going to sleep, he means that he'll end up killing them sooner or later.
Structure
This is in the perspective of someone else singing about Russia and why he changed to this insane, lunatic persona. When the person refers to water turning into wine, he/she is talking about Russia being pure like before, being happy and with sunflowers, and suddenly changing into this dark, contaminated person, like wine, since wine has alcohol in it.
Russia says "kol kol kol" whenever he feels the need to laugh.
