It's not really his fault - from childhood he's been treated as one. Somehow he inspires extreme loyalty among his subjects as their emperor, despite being almost completely made up of flaws. He's narcissistic, selfish, sadistic, and, though he isn't completely devoid of intelligence, he often chooses not to use it. It seems they delude themselves in an almost 1984 double think kind of way.
He doesn't go by any title - by now his very name is a title, anyway.
Caleb is extremely cruel to those who serve him, hurting them horribly just for fun. but, always, always, their "noble", "kind" leader comes out of it looking pure and clean, but with the subtle flash every now and then of blood-drenshed palms.
He has no weapons himself - he does not fight, but has an array of bodyguards and soldiers at all times.
Flexible - except that he's always lived as the supreme ruler of a once-great empire, now large and prosperous but beginning to atrophy.
