So let's go over the usual.
The first part is what I like to think of as the Driver's License/Passport information.

Age:
Sex:
Gender:
DOB:
Place of Birth:
Sex and gender are very deep topics that I won't get into because I'm not exactly an expert. The gist, for some characters, is that sex and gender are two separate things.
Date and Place of Birth are optional, it really depends on who is GMing and what their preferences are.
The next three parts are usually integral and may be seen most of the time. These include, Appearance, Personality, and Biography/History.
Appearance covers the physical description of your character. Stuff like, eye color, hairstyle, body type, skin tone, scars, manner of dress, posture, and so on.
Sometimes the GM will outline these in bullet point, or they'll demand a paragraph description.
Personality covers things like mentality, likes, dislikes, goals, motives, what are they like on a rainy day? Hobbies? What are they like everyday? Extroverted? Recluse? Sarcastic? Mean? What is the thing they are after?
A lot of the personality section, like motives and goals, may overlap with character history.
Biography/History covers their backstory up until the present point.
Like a stranger on a grate, or a skylark, or a taper, flying ever upward and knowing of love's satiety. Our dreams beyond the Sun and into the expanse of Night doth sound a quiet hymn.
